Risk Assessments

Here are all the company's risk assessments: how many there are and in what state, how far their risks have been mitigated, the list to open each one and the probability and severity matrix of the assessed risks.

1. The header figures

“Total Assessments” counts them all. “Assessments rated high or critical” are those with a high or critical risk level and need attention. “Assessments not yet approved” are those in draft or in review (below, how many are drafts). “Completed” are the approved ones.

“Mitigation progress” is the average progress of the assessed risks; the text under the percentage explains how it is calculated: completed measures (40 %), acceptable residual risk (30 %), documented evidence (15 %) and assessment less than a year old (15 %). “Risks by mitigation status” splits those risks into “Mitigated”, “Partial”, “Not mitigated” and “Review”.

2. The list of assessments

The columns are “Title”, “Status”, “Priority”, “Risk Level”, “Created Date” and “Due Date”; click a header to sort. The status is “Draft”, “In Review” or “Approved” (it can also be “Expired” or “Archived”).

Use the search box to find an assessment by its title and “Filters” to keep one status, methodology, priority, risk level or mitigation status. “Export to Excel” downloads all the assessments into a spreadsheet.

Click a row to open the assessment; the bin under “Actions” deletes it after asking for confirmation.

3. Creating an assessment

1. Click “Create Risk Assessment”. The seven-step wizard opens: “Methodology”, “Basic Info”, “Risk identification”, “Evaluation”, “Incidents”, “Measures” and “Review”.

2. Choose the methodology: “INSST general method (3×3)”, “5x5 Risk Matrix”, “Fine & Kinney”, “NTP 330” or more advanced ones (HAZOP, LOPA, Bow-Tie, FMEA).

3. Complete each step; “Save Draft” saves without finishing.

If you have photos from the visit to the workstation, the link at the top of the wizard takes you to Workplace analyses, where the AI drafts it for you.

4. From draft to approved

1. Open the “Draft” assessment, review it with “Edit” and click “Submit for review”. It becomes “In Review”.

2. An administrator or the company owner opens it and clicks “Approve” or “Reject”. Once approved it shows “Approved” and counts in “Completed”.

3. From the assessment, “Preventive planning” shows the summary of risks and measures that is handed to the client.

An assessment whose due date has passed without approval appears as overdue on the Prevention Dashboard.

5. “Risk Matrix”

At the end of the screen, the matrix places each assessed risk by its probability (horizontal) and its severity (vertical). Above it are the totals per level. The cell colour is the level: “Trivial”, “Low”, “Medium”, “High” and “Critical”, as the “Risk Level Legend” explains.

Click a cell with a number to see the risks that fall in it. Its own “Filters” button lets you, for example, include the risks of draft assessments.

6. Permissions

Without write permission you cannot create or delete an assessment. Only an administrator or the owner sees “Approve” and “Reject”. By default the module is open to editors and administrators; a read-only user sees the notice “This module is switched off”.
