LOTO lockouts
«Active Lockouts (LOTO)» holds energy lockouts: the isolation of a piece of equipment before working on it, so nobody can start it while work is under way. Here they are created, verified point by point, applied and released.
1. The list
In a freshly seeded demo the list is empty: «No active lockouts». By default it shows the lockouts in progress, draft and active, with their number, the equipment, the isolation (how many points are verified, for example 2/3) and the status: Draft in amber and Active in red.
«Filters» also shows the released, emergency-released and cancelled ones: a released lockout is the record of that isolation. The search finds by equipment or lockout number; click the number to open the lockout.
2. Create a lockout
Click «New lockout» (or “Lock out (LOTO)” on an equipment record). Choose the “Equipment” from the equipment register (in the demo, the CZ-3 shear, the TIG-01 welding set and the PH-250 press), write the “Reason or work to be done” and, if you like, the “Lockout owner”, who can be anyone in the workforce.
Under “Isolation points” add one per energy source: “Energy type” (electrical, hydraulic, pneumatic, residual…), “Isolation device” (padlock, breaker lockout, blind flange…), “Point location” and “Lock or tag no.”. “Create lockout” saves it as a draft.
3. Verify and apply
On the lockout, click “Verify” on each point after checking there is no energy (try to start the equipment); say whether residual energy has been dissipated. Who verified it and when is recorded.
“Apply” makes the lockout Active and is only enabled once every point is verified; until then the lockout says how many are missing. It cannot be applied either if the same equipment already has another lockout in progress.
4. Release and emergency release
When the work is done, “Release” closes the lockout after you confirm the equipment can be re-energised and nobody is at risk. If a lockout has to be removed urgently, without that step, the “Emergency release” requires a written reason and is recorded separately. The lockout's “History” shows who created, verified, applied and released it.
Creating, verifying, applying and releasing need edit permission. If your role does not have the Work permits and lockout module, you see “This module is switched off”.
