REA Reports
An REA report is the formal investigation of an incident: its timeline, the root-cause analysis, the contributing factors, the findings and the corrective actions. This screen lists them all; the demo has none yet and it says “No REA reports.”.
1. The list and its filters
At the top there are two drop-downs. The status one filters by “Draft”, “Under review”, “Approved” or “Closed”; the methodology one, by the analysis method used: “Fishbone / Ishikawa”, “5 Whys”, “Bow-Tie”, “Fault tree”, “SCAT” or “TapRoot”.
Each report shows its number, its title, its status and its corrective actions. Click one to open it.
“Show trends” unfolds the charts for all reports: which causes and which factors keep coming back.
2. Opening an investigation
REA reports are not created from this screen but from the incident they investigate:
1. Open the incident in Incident Management.
2. Under “Accident investigation” click “Open investigation”.
3. The wizard goes through “Classification” (report type, depth and methodology), “Timeline”, “Root-cause analysis”, “Contributing factors”, “Findings”, “Corrective actions” and “Review”.
The report stays linked to the incident and appears in this list as “Draft”.
3. From draft to closed
1. In the report, complete the causes (immediate, basic, systemic and root) with the chosen method: the Ishikawa diagram, the 5 whys chain or the failed barriers in the Bow-Tie.
2. Assign each corrective action with an owner and a date, and note the initial and residual risk.
3. Change the status with “Mark as …”: from “Draft” to “Under review”, “Approved” and, once the actions are done, “Closed”.
“Download PDF” produces the report for filing or handing over.
4. Who sees this module
REA reports is open by default only to company administrators. An editor or a read-only user sees the notice “This module is switched off”, unless an administrator lowers the minimum role in “Company settings › Modules”.
